A step by step guide to subcutaneous injection.
Step 1 — Get Ready
What you need
- Medication (prefilled syringe)
- New sterile vial
- Needle & syringe (if required)
- Alcohol swabs
- Sharps container
Tip: Lay everything out before you start so you're not fumbling mid-transfer.
Step 2 — Wash Hands
- Clean hands reduce infection risk
- Soap + water or sanitiser
- Dry completely
Step 3 — Inspect your vials
Before opening anything:
- Check the testosterone vial label — name, strength, expiry date
- Check the solution is clear and free of particles or discolouration
- Confirm the sterile vial is sealed and intact
- Do not use if anything looks off
Step 4 — Clean Both Vial Tops
- Remove the plastic flip caps from both vials
- Wipe each rubber stopper with a fresh alcohol swab
- Use a new swab for each vial
- Allow both stoppers to air dry fully (don't blow on them)
Step 5 — Attach Needle to Syringe
- Open the syringe and drawing-up needle while keeping the tips sterile
- Firmly attach the needle to the syringe
- Leave the needle cap on until you're ready to draw
Step 6 — Draw Air into the Syringe
- Pull back the plunger to draw in a volume of air equal to the amount you plan to transfer
- This equalises pressure inside the source vial and makes drawing easier
Step 7 — Inject Air into the Source Vial
- Remove the needle cap
- Insert the needle through the centre of the rubber stopper on the testosterone vial
- Push the plunger down to inject the air
- Keep the needle in the vial
Step 8 — Draw the Testosterone
- Invert the vial so it sits upside down with the needle tip submerged in the oil
- Slowly pull back the plunger to draw your required volume
- Tap the syringe to bring any air bubbles to the top
- Push bubbles back into the vial and re-draw if needed
- Withdraw the needle from the source vial
Step 9 — Transfer into the Sterile Vial
- Insert the needle through the centre of the sterile vial's rubber stopper
- Push the plunger down slowly and steadily
- Allow air to vent naturally as the liquid enters
- Withdraw the needle once the full volume is transferred
Step 10 — Dispose Safely
- Place the used needle and syringe directly into your sharps container
- Do not recap the needle
- Do not reuse needles or syringes
SStep 11 — Label and Store
- Label the new sterile vial with the contents, strength, and date of transfer
- Store as per the original medication instructions (typically room temperature, away from direct light)
- Keep out of reach of children and pets
Key Tips
- Always use a new needle and syringe for each transfer
- Never touch the needle tip or the cleaned rubber stoppers
- Work in a clean, draught-free space
- If you contaminate any equipment, stop and start again with new supplies
- Testosterone is an oil-based solution — draw slowly and patiently
